Your op amp 1 voltages look good, but op amp 2 still shows some odd voltages. With a fresh battery, that IC should show ~4.5V on pins 1 - 3 and 5 - 7. I would suggest referring to the PCB map and schematic for the pedal in
THIS THREAD, about 3/4 of the way down. Note that what you've referred to as "op amp 1" is IC2 on the schematic & PCB map, and your "op amp 2" is actually IC1. I would suggest reflowing the solder joints for all the socket pins for IC1 as well as the solder joints for all the components that connect directly to that IC. While you have the PCB out of the enclosure to do that, take a photo of the solder side of the PCB and post that, please. The reverb module covers a lot of the back of the PCB, but it never hurts to take a look at the solder joints that are visible.
BTW, You may want to recheck the voltage on pin 5 of the reverb module, which should be the same as pin 6 (as it was in your initial voltage readings) and not zero as you've reported here.
