I’m pretty sure the following is correct. Someone correct me if I missed anything.

It appears you have the rotary switch wired wrong. The switch has 2, 3 or 4 positions. The first thing to do is to ensure that you have the switch setup for 3 position operation.

Going by your diagram the switch has poles A, B and C. Each pole has 4 positions (lugs). For Pole A those positions are lugs 1, 2, 3 and 4. Pole B has lugs 5, 6, 7 and 8. Pole C has lugs 9, 10, 11 and 12. Since the switch is being used in 3 position mode lugs 4, 8 and 12 will not be used. You now need to make the following connections. Again, using your diagram, pole A lug 1 gets connected to pole C lug 5. Pole A lug 3 gets connected to pole B lug 11. Pole A goes to eyelet B on the PCB. Pole B goes to eyelet D on the PCB. Pole C goes to eyelet C on the PCB. Pole B lug 10 gets connected to eyelet A on the PCB.
